PROCEDURE:
1. Find out the Public Information Officer( To make it easy, google name of bank & RTI)
2. Draft an application addressing to the PIO( See the sample below)
3. Buy an IPO/DD.
4.Sign the application and send it alongwith DD/IPO to the PIO through registered/speed post
The reply will be sent on the address mentioned on your application within 30 days.
